  1. How do I switch my Straight Medicaid to Carolina Access?

    Contact the Division of Social Services that is in the county in which your Medicaid is assigned and request to be switched over to Carolina Access.

  2. How do I get linked to transportation for medical and mental health appointments?
    If you have Medicaid, contact the Division of Social Services that is in the county in which your Medicaid is assigned and ask for assistance with Medicaid transportation.

  3. How do I get linked to a primary care practice?
    There are several ways to get linked to a primary care practice.  If you have Carolina Access Medicaid, you should be assigned to a primary care practice and the name/contact information for that practice will be listed on the back of your Medicaid card.  If there is not a primary care practice listed on the back of your card, contact the Division of Social Services for assistance with being linked to a primary care practice.  If you have Straight Medicaid, you can utilize this site to locate a primary care practice that will accept Medicaid in your area: http://www.ncdhhs.gov/dma/medicaid/finddoctor.htm.  Prior to making an appointment, please make sure to ask if the provider accepts Medicaid.  Also, if you are assigned to care coordination at Trillium, your care coordinator can assist you in being linked with a primary care practice.

  4. How do I get linked to community resources in my area? 
    To be linked to community resources in a specific area an individual can call 211, visit www.NC211.org or call the Trillium Health Resources Access to Care Line for assistance: 1.877.685.2415.
