Trillium Support Transition And Re-Entry

Trillium’s Support Transition and Reentry (or T-STAR) Support Transition and Reentry) team currently provides support to incarcerated adults who will soon reenter their communities. Most are enrolled in Trillium Tailored Care Management (TCM) and have serious behavioral health conditions. T-STAR also works with some individuals who are not enrolled in Trillium TCM if their Medicaid was suspended solely due to incarceration. 

The T-STAR Team:

  • Coordinates reentry by helping with housing needs, employment guidance, and community inclusion.
  • Links to enhanced behavioral health services, peer support & other community-based services.
  • Links to primary medical care. 
  • Ensures benefits like Medicaid or Social Security Disability are reinstated at release if these benefits were suspended while incarcerated.

The T-STAR program started at Trillium in 2023, and current recidivism rates from preliminary data are very promising.

T-STAR receives referrals directly from the Department of Adult Corrections (NC State Prisons), and works with Trillium members already connected to a Trillium Care Manager.

T-STAR has started a pilot program with the Pitt County Detention Center to take referrals directly from this facility.

The T-STAR Team plans to accept referrals directly from county detention centers in Trillium’s region, probation or parole officers, and others in the future.

Other county detention centers interested in referring incarcerated members directly to the T-STAR program should reach out to T-STAR Director, Erin Kinsel at Erin.Kinsel@TrilliumNC.org to discuss a similar pilot in that region. 

Reentry Simulation Events :

Trillium’s Training Department hosts Reentry Simulation events across our region. These events teach about the many barriers individuals go through when they are released from jail. Participants experience the difficulties of finding a job, keeping a stable home, going to treatment regularly, and following release rules. Upcoming events are posted on our Events and Trainings page.  Registration is free and open to the public.
