Trillium Advantage covers services and settings that may not be covered by typical physical health, behavioral health, or intellectual/developmental disability services such as housing or cell phones. Details on eligibility and how to apply can be read below by clicking on the title.
Value-Added Services

Trillium offers Value-Added Services specifically targeted to Tailored Plan members who experience challenges related to Health Equity in order to improve their access to resources and improve their overall health outcomes.
Members must meet the eligibility requirements for the service and not have any other funding source available to meet the need for the program. Vendor approval is at the discretion of the program administrator. Value-Added Service availability is subject to funding availability. Members with NC Medicaid Direct and State-funded Recipients are not eligible for Value-Added Services.
Please Note:
HOMES, One Time Move-In Assistance, and Transportation are not available to members who live in a Healthy Opportunities Pilot (HOP) county. Find the list of HOP counties here.
Trillium provides up to $75 one time per year for school supplies for covered children enrolled in Pre-K through grade 12. This will often come in the form of a standard school supply kit.

Trillium provides up to $120 for a GED Test Vouchers, once per lifetime, to eliminate any financial barrier relating to testing to obtain a GED.

Trillium will cover up to $250 per year for membership fees for qualifying members who have no other available funding source to meet this need.

HOMES funds can be used to fund person-specific needs to maintain housing such as past due rent, past due utilities, and other housing related needs to maintain safe and stable housing. Members must have an ongoing plan for funding, but Trillium recognizes there are times when members need assistance due to unplanned financial hardship.
Members can contact their care manager or call Member and Recipient Services to ask about HOMES funds.
One Time Move-In Assistance funds can be used to assist members with obtaining permanent housing. The funds can be used to fund person-specific needs to obtain housing such as application fees, rent or utility deposits and other housing related needs to obtain safe and stable housing. Members must have an ongoing plan for funding, but Trillium recognizes there are times when members need assistance due to unplanned financial hardship.
Members can contact their care manager or call Member and Recipient Services to ask about One Time Move-In Assistance funds.
Stay Connected offers one-time, per lifetime, assistance with a cell phone and service plan for one year including talk, text, and data for eligible members ages 18 and older.

Trillium offers access to a mobile app and website to assist with building a 24/7 social support network made up of peers and caregivers. Prior authorization is not required and there is no authorization code needed to utilize the application, as it is available for free to the general public.
Trillium provides up to $50 per member, per year for being active participants in member’s own health. Members can earn $50 for completing their Care Management Comprehensive Assessment (CMCA) with their care manager within 60 days of the beginning of a Tailored Care Management (TCM) assignment.

Trillium provides up to $240 per year, per household, typically in the form of two $30 vouchers per quarter. Voucher options include bus passes, gift cards for ride share agencies, and gift cards for gas depending upon availability in the Trillium catchment area, to help members access the community, obtain food in rural areas, and address their unmet health related resource needs. Members should first use Non-Emergency Medical Transportation prior to accessing this support.

Trillium offers up to $100 one time per year, per child member for after-school sports/activities or youth club memberships (Boys & Girls Club or similar membership) for covered children, ages 6-18.
