Trillium Advantage cubre servicios y configuraciones que actualmente no están disponibles solo para miembros del plan personalizado. Trillium ofrecerá Servicios de Valor Agregado dirigidos específicamente a los afiliados que experimentan desafíos relacionados con la equidad en la salud para mejorar su acceso a estos recursos necesarios y mejorar sus resultados generales de salud. Los afiliados en el Plan Personalizado de Trillium son elegibles para los Servicios de Valor Añadido en todos los condados, excepto en los condados del Programa Piloto de Oportunidades Saludables (HOP, por sus siglas en inglés) cuando el afiliado es elegible y el servicio está disponible en el programa HOP. Los afiliados con NC Medicaid Direct y los beneficiarios de financiamiento estatal no son elegibles para los programas de servicios de valor añadido. Los afiliados no deben tener acceso a ninguna otra fuente de financiamiento disponible para satisfacer la necesidad del programa. La aprobación del proveedor queda a discreción del administrador del programa.
Trillium provides up to $75 one time per year for school supplies for covered children enrolled in grades Pre-K through grade 12.
Members can request assistance through their Care Manager, calling Member and Recipient Services, through their provider, or completing the application that will be available on July 1.
Trillium provides up to $120 for GED Test Vouchers, once per lifetime, to eliminate any financial barrier relating to testing to obtain GED. Members can request assistance through their Care Manager, calling Member and Recipient Services, through their provider, or completing the application that will be available on July 1.
Members who exercise regularly are generally more likely to have better health outcomes. Trillium will cover the annual cost (up to $250) of gym memberships to help people to stay active and well. Eligible individuals must not have a benefit (i.e. Innovations Waiver or TPL) that would cover a gym membership, or not have access to other available funding source. Members can request assistance through their Care Manager, calling Member and Recipient Services, through their provider, or completing the application when the application is available on July 1.
Trillium provides up to $50 per year for being active participants in member’s own health. Members can earn $50 for completing their CMCA with their Care Manager within 60 days of the beginning of a Tailored Care Management Authorization. Members can request assistance through their Care Manager and then complete the application that will be available on July 1.
Trillium will provide up to $200 in vouchers for healthy foods for completing healthy eating and lifestyle classes. $50 vouchers are available up to 4 times per year, per household. Members must complete the following healthy lifestyle/eating courses in the Member My Learning Campus to receive the rewards:
- Healthy Eating
- Healthy Eating Tips
- Managing Stress
- Take Control of Your Health and Wellness
Members can find the online classes at My Learning Campus and get the gift cards through Care Management staff, calling Member and Recipient Services, through their Provider, or by completing the application when available on July 1.
Having permanent housing is important for improved health outcomes. Trillium offers HOMES grants to assist members with finding and keeping permanent housing. The funds can be used for person-specific needs, such as application fees, deposits (rental and/or utilities), past due rent, past due utilities, and pest extermination. Please speak to your care manager or call Member and Recipient Services to ask about HOMES funds.
Trillium recognizes that health does not just happen within in the walls of provider offices. Addressing unmet-health related resource needs such as food or transportation helps improve overall health.
Trillium’s Neighborhood Connections team assists individuals and families in finding local resources to help with these needs.
Eligible members may not have access to any other available funding sources to meet this need.

Members who use the app "I Am Sober" will have a support network, help tracking goals and time sober, daily pledges and workbook activities, and an automatic discount applied to online therapy through the app.
It is important to stay in touch with your Provider or Care Manager to get the care you need. Stay Connected helps you get a free cell phone for one year with unlimited talk, text, and 10 gigabytes of data. The member must be 18 years of age or older. To see if you are eligible for Stay Connected, please reach out to your Care Manager, your Provider, call the Member and Recipient Services or fill out this application below.
Having transportation is important to addressing physical health, behavioral health, and unmet health-related resource needs such as food and employment. Providing transportation to our members will help them get to health care appointments, buy food at stores, and pick up prescriptions.
Trillium provides up to $60 per quarter, per household, typically in the form of two $30 vouchers each quarter up to four times per year) that can include bus passes, gift cards for ride share agencies such as Uber or Lyft, and gift cards for gas depending on the availability in the Trillium catchment area. Members should first use Non-Emergency Medical Transportation prior to accessing this support.
Members can ask for this voucher through their Care Manager or by calling Member and Recipient Services.
Trillium offers $100 one time per year per child member aged 6-18 for after-school youth club membership in a Boys & Girls Club or similar after-school sports/activities. Members can request assistance through their Care Manager, calling Member and Recipient Services, through their Provider, or completing the application that will be available on July 1.
